29 CFR 1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system. A. A & A Home Improvement worksite located at the Saddlebrooke Subdivision Lot 195, Perrysburg, Ohio: On or about April 24, 2014, the employer failed to ensure employees were protected from a falls while working from a ladder jack scaffold. The employee was installing vinyl siding on the West side of the two story residential structure exposed to a fall distance of approximately 19.00 feet.

29 CFR 1926.454(a) The employer did not have each employee who performs work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards. A. A & A Home Improvement worksite located at the Saddlebrooke Subdivision Lot 195, Perrysburg, Ohio: On or about April 24, 2014, the employer failed to ensure employees working on a ladder jack scaffold were trained to recognize and avoid hazards associated with this type of scaffold including but not limited to, fall protection.
