29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees: (a) Floor; a maintenance employee was installing new bumpers on a gate stop on the DBCS 990 Series letter sorting machine, machine no. 32, without powering down and locking out the machine. The plexiglass doors on the front of the machine were open to perform the task, however the disconnect plugs can be pushed in and out to defeat their safety feature.
