29 CFR 1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system.  a) Jobsite: South side of Germantown Ave. & E Penn St - On or about 4/14/14, employees working on a ladder jack scaffold were not protected from falling by the use of a personal fall arrest system.  Employees were capping window frames approximately 12 feet above ground level.

29 CFR 1926.501(b)(14): Each employee working on, at, above, or near wall openings (including those with chutes attached) where the outside bottom edge of the wall opening is 6 feet or more above lower levels and the inside bottom edge of the wall opening is less than 39 inches above the walking/working surface, was not protected from falling by the use of a guardrail system, a safety net system, or a personal fall arrest system.  a) Jobsite: South wall of the 2nd floor - On or about 4/14/14, no fall protection system was used to protect employees from falling off the open wall openings on the 2nd floor of the commercial/residential building. Employees were capping window frames approximately 12 feet above ground level.
