29 CFR 1910.134(e)(1): The employer did not provide a medical evaluation to determine the employee's ability to use a respirator, before the employee was fit tested or required to use the respirator in the workplace:  (a) Sandblasting Area - Employees who were required to wear a "Bullard" hood while sandblasting did not have a medical evaluation performed by a physician or other licensed health care professional.  Employees sandblasted, on or about March 24, 2014.  (b) Welding and Fabrication Shop - Employees who were required to wear 3M" Adflo" Powered Air Purifying Respirators (PAPR) while welding in confined spaces or on stainless steel did not have a medical evaluation performed by a physician or other licensed health care professional. Employees welded with the PAPR, on or about April 17, 2014.  Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1910.242(b): Compressed air used for cleaning purposes was not reduced to less than 30 PSI:  (a) Table Mill - An employee used a blow down gun for cleaning that delivered 60 PSI at the nozzle, thus exposing employees to embolism hazards, on or about April 23, 2014.                                                                           No abatement certification or documentation required.

29 CFR 1910.134(i)(5)(iv): The tag containing the most recent change to the sorbent beds was not kept or maintained at the compressor:    (a) Sandblasting Area - The inline filter for the breathing air to the "Bullard" sandblasting hood did not have a tag with the date of the most recent filter change.  Employees used the sandblasting hood, on or about March 24, 2014.   Abatement certification required within 10 days after abatement date. 29 CFR 1910.179(j)(3): Complete inspections of the crane was not being performed at intervals as generally defined in paragraph (j)(1)(ii)(b) of this section:  (a) Penn Iron Works, Sinking Spring Pennsylvania - Employees used overhead cranes to move materials and welded assemblies around inside the building from one operation to another.  The employer did not ensure that a periodic inspection of the cranes was conducted at least every 12 months, on or about April 24, 2014.  Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1910.1200(h)(3)(iv):   The employer did not produce a hazard communication program that included an explanation of the labels received on shipped containers and the workplace labeling system used by their employer and the safety data sheet, including the order of information and how employee could obtain and use the appropriate hazard information:  (a) Penn Iron Works, Sinking Spring Pennsylvania - The employer did not provide employees with training on the revised hazard communication standard (HCS 2012) label elements and the new format and type of information on each section of the safety data sheets (SDSs) to ensure that when employees begin to see HCS 2012 compliant labels and SDSs in their workplace, employees understand how to use the labels and SDSs and access the information effectively, on or about April 23, 2014.   Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
