29 CFR 1910.132(d)(1): The employer did not assess the workplace to determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment (PPE): Employees engaged in roof top exhaust fan maintenance activities from a 4:12 pitch roof were exposed to fall hazards of approximately 8 feet to the ground below in that a hazard assessment had not been conducted in order to determine what personal protective equipment is needed when engaging in such tasks.
