29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury:           a) Wilmer Javier Urbina @ 1767-1787 Sherman Dr., St. Charles, MO 63303: On April 18, 2014, the employer did not ensure that employees were protected from flying debris while using pneumatic nailers while conducting residential roofing work. This condition exposed employees to struck-by eye hazards.

29 CFR 1926.1053(b)(1): The employer did not ensure that when portable ladders were used to access the upper landing of the building the ladder side rails were not extended at least 3ft above the upper landing surface:       a) Wilmer Javier Urbina @ 1767-1787 Sherman Dr., St. Charles, MO 63303: On April 18, 2014, the employer did not ensure that ladders that were used to access a roof was at least 3 ft. above the landing surface. Two ladders were observed and each was raised approximately 1 rung or 1 ft. above the landing surface. Employees were observed using both ladders. This condition exposed employees to a fall hazard in excess of 25 ft.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor was an alternative fall protection plan which meets the requirements of 1926.502(k) developed and implemented:         a) Wilmer Javier Urbina @ 1767-1787 Sherman Dr., St. Charles, MO 63303: On April 18, 2014, the employer did not ensure that fall protection systems were utilized by employees engaged in roofing work. Employees worked on a 6:12 pitch, three-story, multi-family unit, residential roof. This condition exposed employees to a fall hazard in excess of 25 ft.         Wilmer Javier Urbina was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1926.501(b)(13), which was contained in OSHA inspection number 904186, citation number 1, item number 3 and was affirmed as a final order on August 29, 2013, with respect to a workplace located at 1455-1467 Brentwood, MO 63144.
