On or about April 24, 2014, at the above addressed jobsite, employees were exposed to a fall hazard while aligning roof trusses and securing a wall from the edges of a residential home under construction.

On or about April 24, 2014, at the above addressed jobsite, employees who were exposed to fall hazards were not trained in the use of personal harnesses, lanyards, and fall protection systems prior to being allowed to work from roof trusses and edges of a residential home under construction.

On or about April 24, 2014, at the above addressed jobsite, employees were exposed to being struck-by a portable circular saw that was left on the edge of a wall with its electrical cord hanging from the wall while the employer did not conduct frequent and regular inspections of the construction site.
