29 CFR 1910.213(i)(1): Non-working portions of the blade of bandsaws were not enclosed or guarded: (a) On or about April 22, 2014 the inspection revealed that employees were potentially exposed to serious injury from the Rockwell Model 14 vertical band saw that did not have a guard over all non-working portions of the blade. The bandsaw was located in the warehouse carpentry area at 1300 Airport Lane in North Bend, Oregon. (b) On or about April 23, 2014 the inspection revealed that employees were potentially exposed to serious injury from the MAMB 128 horizontal bandsaw that did not have a guard over all non-working portions of the blade. The bandsaw was located at the Dean Creek Recreation Maintenance shop.

29 CFR 1910.242(b): Compressed air used for cleaning purposes was not reduced to less than 30 psi: (a) On or about April 23, 2014 the inspection revealed that employees were potentially exposed to serious injury from a compressed air nozzle that was not designed to reduce the pressure to less that 30 psi. The air nozzle was located in the Dean Creek Maintenance Shop at the north wall by the air compressor.

29 CFR 1910.305(b)(2)(i): Each outlet box in completed installations did not have a cover, faceplate, or fixture canopy: (a) On or about April 22, 2014 the inspection revealed that employees were exposed to injury such as minor burns or electric shock from exposed live parts due to an outlet that did not have a faceplate. The outlet was located under the circuit breaker panel on the northwest wall of the warehouse at 1300 Airport Lane in North Bend, Oregon.

29 CFR 1926.601(b)(14): All defects were not corrected before motor vehicle(s) were placed in service: (a) On or about April 22, 2014 the inspection revealed that employees were potentially exposed to serious injury from the use of the Superior Broom brush cleaner, Model DT80, without a working horn. The Superior Broom was in use near the intersection of Lower Smith Road and Vincent Creek Road at the time of the inspection. (b) On or about April 23, 2014 the inspection revealed that employees were potentially exposed to serious injury from the use of backhoe that did not have a working backup alarm. The backhoe was in use on the beach at the Loon Lake Recreation Site at the time of the inspection. The Bureau of Land Management was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 331945, notice number 1, item number 3 and was affirmed as a final order on July 23, 2012, with respect to a workplace located at North Bend, Oregon.

29 CFR 1910.304(f)(1)(viii): Circuit breakers used as switches in 120-volt and 277-volt, fluorescent lighting circuits were not listed and marked "SWD." (a) On or about April 22, 2014 the inspection revealed that employees used circuit breakers to switch lights on and off daily and that the breakers were not rated for this use. Located on the northwest wall of the warehouse building at 1300 Airport Lane in North Bend, Oregon.
