(a) Location: 310 Brown Street, Tewksbury MA, 01876 On or about 4/25/2014, a competent person did not perform a frequent and regular inspection of the job-site to identify safety and health hazards such as: Damaged ladders, damaged power extension cords, damaged scaffold plank and working in close proximity to power lines.
