29 CFR 1926.451(e)(1): When scaffold platforms are more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface shall be used: a) On or about April 24, 2014 - at the jobsite located at 6750 S. Stony Island Avenue, Chicago, IL - employees performing tuck pointing activities from a tubular welded frame scaffold approximately twelve (12) feet above ground level were not provided with a ladder to safely access the work level, thereby exposing employees to fall hazards.

29 CFR 1926.501(b)(1): "Unprotected sides and edges." Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level shall be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: a) On or about April 24, 2014 - at the jobsite located at 6750 S. Stony Island Avenue, Chicago, IL - employees performing masonry work on a flat roof were exposed to falls due to the presence of unprotected sides/edges, thereby exposing the employees to fall hazards of approximately forty (40) feet.
