29 CFR 1926.503(a)(1): The employer shall provide a training program for each employee who might be exposed to fall hazards. The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards.  At the jobsite exposed employees were unable to recognize the following hazards:     - Unguarded floor hole(s).     - Unguarded wall openings.     - Unguarded Open sided edges.

The program shall enable each employee to recognize hazards related to ladders and stairways, and shall train each employee in the procedures to be followed to minimize these hazards.  + At the job site employees were unable to recognize the following hazards:     - Ladder was not extended 3 feet above the landing/eave surface.     - Lack of stair rails on the stairway with 7 risers     - Damaged ladder legs (foot)

29 CFR 1926.501(b)(13): "Residential construction." Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure. Exception: When the employer can demonstrate that it is infeasible or creates a greater hazard to use these systems, the employer shall develop and implement a fall protection plan which meets the requirements of paragraph (k) of 1926.502. Note: There is a presumption that it is feasible and will not create a greater hazard to implement at least one of the above-listed fall protection systems. Accordingly, the employer has the burden of establishing that it is appropriate to implement a fall protection plan which complies with 1926.502(k) for a particular workplace situation, in lieu of implementing any of those systems.  At the job site, employees framing a multi-story residential home were exposed to the following fall hazards:     a) On the first floor level, a floor hole with 15 feet long X 3 feet wide X 9 feet deep. b) On the fist floor level, two floor holes with 2 feet long X 1 foot wide X 9 feet deep. c)  On the first floor level, one unguarded window opening with 11 feet fall distance. d) On the first floor level, one unguarded window opening with 6 feet fall distance. e) On the second floor level six unguarded windows each with 18 feet fall distance.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed.  At the job site employees were utilizing a portable ladder to descend from the 2nd floor to the 1st floor. The ladder side rails extended approximately 1ft. beyond the landing, but less than 3ft as required. Employees were exposed to approximately an 11ft. fall hazard.

29 CFR 1926.1053(b)(17): Fixed ladders with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, or corroded components, shall be withdrawn from service until repaired. The requirement to withdraw a defective ladder from service is satisfied if the ladder is either:  At the job site, the portable ladder used too access the second floor had both slip resistant footing missing. The workers were exposed to falling while using the ladder.
