Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level shall be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. a. On or about April 25, 2014 - at the above addressed jobsite, employees conducting tuck pointing and parapet repair, approximately forty (40) feet above the ground, were exposed to falls created by the presence of unguarded sides and edges.

29 CFR 1926.502(d)(15): Anchorages used for attachment of personal fall arrest equipment shall be independent of any anchorage being used to support or suspend platforms and capable of supporting at least 5,000 pounds (22.2 kN) per employee attached: a. On or about April 25, 2014 - at the above addressed jobsite, an employee conducting debris removal from a roof, approximately forty (40) feet above the ground, attached the personal fall arrest system to the support of a scaffold platform, thereby exposing the employee to fall hazards.
