29 CFR 1926.501(b)(3):  Each employee in a hoist area was not protected from falling 6 feet (1.8 m) or more to lower levels by guardrail systems or personal fall arrest systems:      a)  Employees were exposed to the hazard of falling approximately 11.4 feet to the ground while working in the hoist area.    No abatement certification or documentation is required for this item.

29 CFR 1926.1053(b)(1): 29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigged support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder:       a)  An extension ladder used by employees to gain access to the working surface was not long enough to have it's side rails extend at least 3 feet above the surface.     No abatement certification or documentation is required for this item.
