Employees were exposed to struck-by hazards from falling objects positioned above head level, as observed on April 28, 2014. 29 CFR 1910.178(a)(5): The powered industrial truck was equipped with front-end attachments other than factory installed attachments, however the employer did not request that the truck be marked to identify the attachments and show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered: a) Banding Area/Shipping Yard, 109 Industrial Park Road, Beech Creek - Operators used the Ingersoll Rand Rough Terrain Forklift Model RT708H (Serial No. 161417 SII) to elevate an employee in a front-end attachment, specifically a personnel platform fabricated on-site by company employees without knowledge of the weight and limitations of the attachment. Employees were exposed to struck-by hazards by the lack of markings regarding the attachment that created the potential for operators to be unaware of changes to the truck's lifting capacity and center of gravity, as observed on April 28, 2014. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation.
