29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section.  Jobsite: The employees, that were working on the fabricated frame scaffold were exposed to a fall hazard and were not protected from the fall hazard by the use of appropriate fall protection equipment.

29 CFR 1926.602(c)(1)(vi): Industrial truck(s) in use did not meet the applicable requirements of design, construction, stability, inspection, testing, maintenance, and operation, as defined in American National Standards Institute B56.1-1969, Safety Standards for Powered Industrial Trucks.  Jobsite:   1. Employees were permitted to walk and work under a supported load on a telehandler, exposing themselves to a struck by hazard.  2. The operator of the Telehandler left the controls of the machine without fully lowering the load with the controls neutralized and the power shut off.
