(b) On or about 3/25/14, Warehouse Shipping Clerk - a temporary employee strained his right wrist which resulted in days away from work. (a) On or about 2/8/13, Shipping Employee - an employee strained his back which resulted in days away from work but was recorded as job transfer or restrictions and the object/substance that directly injured or made the person ill was not identified. (b) On or about 8/12/13, Shipping Employee - an employee strained his back which resulted in days away from work but was recorded as job transfer or restrictions and the object/substance that directly injured or made the person ill was not identified.
