29 CFR 1904.4(a): The employer did not record each work-related fatality, injury or illness case that resulted in the general recording criteria on the OSHA Form 300 or equivalent.   On or about 4/28/14 - at 3242 Moody Parkway, Moody, AL, the employer did not record the following workplace injuries on the OSHA 300 Log for the calendar year 2014.  (a) On or about 3/13/14, Warehouse Shipping Clerk - a temporary employee strained his back which resulted in days away from work.  (b) On or about 3/25/14, Warehouse Shipping Clerk - a temporary employee strained his right wrist which resulted in days away from work.  (c) On or about 4/8/14, Shipping Employee - an employee strained his back which resulted in days away from work.  (d) On or about 4/18/14, Shipping Employee - an employee strained his back which resulted in days away from work.

29 CFR 1904.32(a)(1): A review of the OSHA Log of Recordable Work-Related Injuries and Illnesses (OSHA Form 300 or equivalent) was not completed to ensure the Log was accurate and complete, and/or deficiencies were not identified and corrected at the end of each calendar year.  On or about 4/28/14 - at 3242 Moody Parkway, Moody, AL, the employer did not ensure that the OSHA 300 Log for the calendar year 2013 was accurate and complete.  (a) On or about 2/8/13, Shipping Employee - an employee strained his back which resulted in days away from work but was recorded as job transfer or restrictions and the object/substance that directly injured or made the person ill was not identified.  (b) On or about 8/12/13, Shipping Employee - an employee strained his back which resulted in days away from work but was recorded as job transfer or restrictions and the object/substance that directly injured or made the person ill was not identified.
