29 CFR 1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns: a) First Level, Front Rear Center - A worker was working on a floor/ceiling joist above workers walking and working on first level who were not protected from possible falling objects, on or about 4/28/2014. Abatement certification required within10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): a) Top Right Side, Rear - Worker nailing plywood to first level floor/ceiling joist was not protected from falling 8' to the plywood level below, on or about 4/28/2014. b) First Level Rear, Left Wall - Workers framing walls around an 8' x 6' floor to ceiling wall opening were not protected from falling 8' to the ground and scrap materials below, on or about 4/28/2014. c) First Level, Right Side Floor - Workers framing walls around an 7'x 4' floor opening were not protected from falling 7' to the concrete floor below on or about 4/28/2014. Abatement certification required within10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
