Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment that was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to laceration hazards, puncture hazards, and eye injuries:    a) For the Green Heron Maintenance, Inc. facility at 12700 Dewey Rd., Bonita Springs, FL 34135, as observed on or about 04/24/2014: employees were exposed to laceration/puncture hazards or eye injuries associated with compressed air that was conveyed through PVC piping in the maintenance/tool shop. PVC air lines were observed to run throughout the shop and were not encased in shatter resistant material. This resulted in potential puncture/laceration wounds to employees from the shattering of pressurized PVC air lines.

29 CFR 1910.243(c)(1): Abrasive wheel(s) were used on portable grinder(s) which were not provided with safety guard(s) meeting the requirements specified in 29 CFR 1910.243(c)(1) through (c)(4)(a):  a) For the Green Heron Maintenance establishment at 12700 Dewey Rd., Bonita Springs, FL, as observed on or about 04/24/2014, employees used a five inch portable grinder without its safety guard. Employees were exposed to hazards of contacting unguarded portable abrasive grinding wheels or being struck by fragments should the abrasive wheel shatter.
