29 CFR 1910.212(a)(1): Machine guarding was not provided to protect operator(s) and other employees from hazard(s) created by: a. On or about February 24th, 2014 at the above addressed site, an employee was exposed to amputation/laceration hazard while operating a band saw with the guard in an upright position. b. On or about February 24th, 2014 at the above addressed site, an employee was exposed to amputation/laceration hazard while operating a panel saw with no guard in place to protect the employee from the rotating saw blade. c. On or about February 24th, 2014 at the above addressed site, an employee was exposed to amputation/laceration hazard while operating a panel saw without a guard in place for an exposed chain and sprocket.
