29 CFR 1910.145(c)(3): Safety instruction signs were not present where there is a need for general instructions and suggestions relative to safety measures:    (a) 301 Chestnut St., East Longmeadow, MA: On or about April 29, 2014, safety instruction signs were not used to warn employees against the potential for injuries and the practices to be followed (such as glove use) during unjamming operations which are performed while the Greenerd 15 ton hydraulic press(s) are in operation at this workplace.      Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29 CFR 1903.19(d)(1).

29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and/or utilized for the control of potentially hazardous energy:     (a) 301 Chestnut St., East Longmeadow, MA: On or about April 29, 2014, an employee was clearing a jam in the Greenerd 15 ton hydraulic press when the machine cycled and he was struck by a finished reciprocating saw blade which lacerated the palm of his right hand and a procedure was not developed, documented and/or utilized for the control of hazardous energy during this process.      Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29 CFR 1903.19(d)(1).
