29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toe boards, safety net systems, or personal fall arrest systems: a) For employees working at 8260 Cypress lake Drive in Fort Myers, FL, as observed on or about April 28, 2014. Employees were removing and installing asphalt roofing shingles on a roof with a 7:12 pitch. The roof had an eave height of 10-feet 3-inches and the employees had no fall protection system in place.

Employees were using an electric blower to clean off the roof that was not protected by a working ground fault circuit interrupters for personal protection.

29 CFR 1926.403(b)(1)(iii): Safety of equipment was not determined on the basis of electrical insulation: a) For employees working at 8260 Cypress Lake Drive in Fort Myers, FL, as observed on or about April 28, 2014. Employees were using an electric blower to clean off the roof that had damaged and removed electrical insulation on the cord. The damaged part of the cord was wrapped with electrical tape that did not have the same insulation properties as the original cord.
