Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system. a) for employees working at 690 South Tamiami Trail in Nokomis (FL), as observed on or about April 28, 2014. Employees were installing metal roofing panels on a roof with 2 pitches of 0:12 on the main roof and 12:12 on the eave edge roof. The roof had an eave height of 20 feet and the employees had no fall protection in place.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladders length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: a) for employees working at 690 South Tamiami Trail in Nokomis (FL), as observed on or about April 28, 2014. Employees were using an extension ladder to access the roof that was not extended a minimum of 3 feet above the upper landing surface. The extension ladder was extended approximately 12 inches above the upper landing surface that was 18 feet above the ground.

Employees were using a stairway that was not equipped with at least one handrail. Employees were using a stairway that had 8 risers to the first landing and another 8 risers to the second landing with no handrails installed. The height to the first landing measured 5 feet.
