On or about March 13, 2014 and at time prior thereto, employee(s) were observed performing bridge work over water from an aerial lift and ring buoys were not provided for emergency rescue operations.  The employee(s) were exposed to a potential drowning hazard.

On or about March 13, 2014 and at times prior thereto, employee(s) were observed performing bridge work from an aerial lift while suspended over the Canadian River and a lifesaving skiff was not immediately available for rescue operations.  The employee(s) were exposed to a potential drowning hazard.
