29 CFR 1926.501(b)(12): Each employee engaged in the erection of precast concrete members (including, but not limited to the erection of wall panels, columns, beams and floor and roof "tees") and related operations such as grouting of precast concrete members, who is 6 feet (1.8m) or more above lower levels were not protected from falling by guardrail systems, safety net systems, or personal fall arrest systems. On or about August 10, 2021, the employer did not ensure the horizontal lifeline was properly secured to the anchor. Employees were installing a concrete beam at a height greater than six (6) feet and this exposed the employees to a fall hazard to the concrete ground and construction debris below.

On or about August 10, 2021, the employer did not ensure the horizontal lifeline was installed and used under the supervision of a qualified person. This exposed an employee in use of the horizontal lifeline to a fall hazard.
