29 CFR 1926.350(a)(10): Oxygen cylinders in storage shall be separated from fuel-gas cylinders or combustible materials (especially oil or grease), a minimum distance of 20 feet (6.1 m) or by a noncombustible barrier at least 5 feet (1.5 m) high having a fire-resistance rating of at least one-half hour. BARGE, NEXT TO THE DOGHOUSE: Oxygen cylinders were stored together with tanks of Acetylene and Propane.

29 CFR 1926.605(b)(2): Unless employees can step safely to or from the wharf, float, barge, or river towboat, either a ramp, meeting the requirements of paragraph (b)(1) of this section, or a safe walkway, shall be provided. BARGE: Employees were accessing/egressing the barge deck using a 16 foot long, 2 X12 wooden plank that was not secured in place to prevent accidental displacement and no handrails or equivalent means were provided.

BARGE: The employees, exposed to falls while walking/working on the edge of the barge were not wearing work vests, buoyant vests and/or equivalent that were approved by the United States Coast Guard (USCG).
