29 CFR 1910.178(l)(1)(i): The employer shall ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l).    WORKSITE: The employer did not ensure through a written certification that the operator of the GEHL model RS6-42 rough terrain forklift was trained and evaluated to operate the forklift safely.

WORKSITE: The operator of the GEHL model RS6-42 rough terrain forklift was not positioned at or near the controller of the forklift while the forks were elevated and the engine was running.

29 CFR 1926.501(b)(13): "Residential construction." Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure.    WORKSITE: The employee installing sheathing on the 8/12 pitch roof, 29 feet, 6 inches above the ground was not protected from the hazard of falling by the use of guardrail systems, safety net systems or personal fall arrest systems.      Pyramid Framing LLC was previously cited for a violation of an equivalent occupational safety and health standard 29 CFR 1926.501(b)(14), which was contained in OSHA inspection number 315338178 citation number 1, item number 1a, issued on 7/22/2011 and affirmed as a final order on 8/19/2011 with respect to a workplace located at 126 Woodrow Avenue, Fairfield, CT.

Flexible cords shall be connected to devices and fittings so that strain relief is provided which will prevent pull from being directly transmitted to joints or terminal screws.  WORKSITE: The following extension cords, used to energize power tools, such as (but not limited to) portable circular saw and a battery charging station had the sheathing pulled away from the plug heads, exposing the internal wiring:  1.     The orange extension cord. 2.     The orange/black extension cord.
