29 CFR 1926.451(b)(5)(i): Each end of a platform 10 feet or less in length shall not extend over its support more than 12 inches (30 cm) unless the platform is designed and installed so that the cantilevered portion of the platform is able to support employees and/or materials without tipping, or has guardrails which block employee access to the cantilevered end. WORKSITE: The ends of two (2) scaffold planks placed on the fabricated frame scaffold as part of the supported scaffold extended between 18 inches and approximately 3 feet beyond the frame and the planks were not secured so that the cantilevered portion of the planks would support materials and employees without tipping.

29 CFR 1926.451(g)(1)(vii): For all scaffolds not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, each employee shall be protected by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section. WORKSITE: The employees working on the following scaffolds were not protected from the hazard of falling by the use of appropriate fall protection systems, exposing the employees to falls between 14 feet, 6 inches and 17 feet, 3 inches to the ground below. 29 CFR 1926.451(e)(1): When scaffold platforms are more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface shall be used. Crossbraces shall not be used as a means of access. WORKSITE: The employees working from the fabricated frame and supported scaffolds were not provided with appropriate access to/from the scaffolds forcing the employees to climb both the end frames and the cross bracing.

29 CFR 1926.454(a): The employer shall have each employee who performs work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards. WORKSITE: The employees, using scaffolds during the building of a chimney were not provided with information and training in the type(s) of scaffolds being used and the procedures necessary to eliminate or minimize those hazards.
