29 CFR 1910.147(c)(6)(i): The employer did not conduct an annual or more frequent inspection of the energy control procedure to ensure that the procedure and requirements of this standard were followed:    Manitowoc Lifts and Manufacturing, LLC did not evaluate the lock out procedures at periodically (at least once per year), thereby exposing employees to amputation hazards.    Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR 1910.147(c)(8): Lockout or tagout was not performed only by the authorized employees who are performing the servicing or maintenance:  Southeast, MLM shop, fabrication machines: on April 30, 2014, each authorized employee performing servicing and maintenance tasks did not perform his lockout/tagout procedures individually for exclusive control and prevention of unexpected and unauthorized energization.  Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR 1910.303(g)(2)(i): Except as elsewhere required or permitted by Subpart S of Part 1910, live parts of electric equipment operating  at 50 volts or more were not guarded against accidental contact by use of approved cabinets or other forms of approved enclosures or by any of the means identified in paragraphs (A), (B), (C), and (D) of 29 CFR 1910.303(g)(2)(i):  (a) Southeast, MLM shop, lathe #7 overhead lighting fixture: on April 30, 2014, employees were exposed to electric shock and fire hazards where a connecting flexible cable's plug and outer jacket were damaged and wrapped with black tape.  (b) Southeast, MLM shop, lathe #7 overhead lighting fixture: on April 30, 2014, lighted linear fluorescent tube light bulbs were not guarded against accidental damage and contact where located below 8 ft. above the adjacent floor.   There is no abatement certification or documentation required for this item.

29 CFR 1910.305(b)(2)(i): Pull boxes, junction boxes, and fittings were not provided with covers approved for the purpose:  Southeast, MLM shop, lathe #7 duplex outlet for overhead lighting fixture: on April 30, 2014, employees were exposed to electric shock and fire hazards where energized conductors were exposed because of a missing face plate on a duplex outlet connecting flexible cable for overhead light.   There is no abatement certification or documentation required for this item.
