29 CFR 1910.37(a)(3): The exit access must not go through a room that can be locked, such as a bathroom, to reach an exit or exit discharge, nor may it lead into a dead-end corridor:  In the Armor Systems Test Branch YTC Maintenance, Building 2210  Exit access, from the work floor to the exterior exit discharge door, went through the Armor Systems Test Office. The side-hinged door, from the work floor into the office, could be locked with a key. There was no panic bar on the door. The length of building was about 150 feet along a north to south axis, and the width was about 60 feet. The door to the office was signed as the exit for north end of the building. The only other way out of the north end of the building was through a 16 foot high by 16 foot long roll-up door. Exit discharge for the south end of the building was located about 150 feet from the north side of building through a side-hinged door next to a 16 by 16 foot roll-up door. Employees in the building were exposed to the hazards of fire.

29 CFR 1910.151(b): In the absence of an infirmary, clinic, or hospital in near proximity to the workplace which was used for the treatment of all injured employees, a person or persons were not adequately trained to render first aid:  At the remotely located test ranges, where the Gunners Shop, Building 3522, employees fired weapons  Employees adequately trained in first aid were not always available. Some of the test ranges were located up to one hour of travel time from emergency medical services. Employees have incurred amputation injuries on the test ranges while firing mortars. Employees were exposed to the hazard of being struck by the projectiles fired from the weapons on the test ranges.

29 CFR 1904.40(a): When an authorized government representative asked for the records kept under Part 1904, the employer did not provide copies of the records within four (4) business hours:  At the establishment - At about 10:05 am on Wednesday, April 30, 2014, an OSHA compliance officer asked for the OSHA 300 logs for 2009 through 2013. The OSHA 300 log for 2013 was provided at about 2:15 pm on Monday, May 5, 2014, and the logs for 2009 through 2012 were not available.
