29 CFR 1926.403(b)(2): Listed, labeled, or certified equipment was not installed and used in accordance with instructions included in the listing, labeling, or certification:  On May 1, 2014, at Lot 16 unit Apartment Project on the ground in the front yard of the main building, employees were using a household relocatable power tap not approved for outdoor use or construction sites for providing power to portable tools and exposed employees to electrical shock hazards.

29 CFR 1926.404(b)(1)(i): Employer did not use either ground fault circuit interrupters as specified in paragraph (b)(1)(ii) of this section, or an assured equipment grounding conductor program as specified in paragraph (b)(1)(iii) of this section to protect employees on construction sites.   On May 1, 2014, at Lot 16 unit Apartment Project on the ground in the front yard of the main building, employees were using extension cords to provide power to portable tools on the construction site without using a ground fault circuit interrupter and exposed employees to electrical shock hazards.

29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:        On May 1, 2014, at the Lot 16 unit Apartment Project on the east side of the building, an employee standing and working from a scaffold was not protected from falling 12 feet to the lower level.
