29 CFR 1910.23(c)(1)(i):  Every open-sided floor or platform 4 feet or more above adjacent floor or ground level shall be guarded by a standard railing (or the equivalent as specified in paragraph (e)(3) of this section) on all open sides except where there is entrance to a ramp, stairway, or fixed ladder. The railing shall be provided with a toeboard wherever, beneath the open sides, Persons can pass,  a. In the shop, on the mezzanine there was an open sided floor, that was not guarded with a guard rail system. Employees were exposed to an 11 foot fall hazard.

The employees handle vehicle batteries as part of the truck maintenance and repair aspect of the shop, employees are not provided with an eye wash station, thereby employees are exposed to contact with corrosive materials hazard.

29 CFR 1910.215(b)(9): Exposure adjustment. Safety guards of the types described in Subparagraphs (3) and (4) of this paragraph, where the operator stands in front of the opening, shall be constructed so that the peripheral protecting member can be adjusted to the constantly decreasing diameter of the wheel. The maximum angular exposure above the horizontal plane of the wheel spindle as specified in paragraphs (b)(3) and (4) of this section shall never be exceeded, and the distance between the wheel periphery and the adjustable tongue or the end of the peripheral member at the top shall never exceed one-fourth inch. (See Figures O-18, O-19, O-20, O-21, O-22, and O-23.)  a. In the workshop area, the Craftsman bench grinder did not have a tongue guard. Employees were exposed to a struck by hazard.

Unless specifically permitted otherwise in paragraph (g)(1)(ii) of this section, flexible cords and cables may not be used: As a substitute for the fixed wiring of a structure;  a. In the workshop, the employer hard wired a flexible cord into a receptacle and used the plug end to plug into a second receptacle to energize the receptacle that was hard wired. Employees were exposed to a fire hazard.

However, rosettes and cleat-type lampholders and receptacles located at least 2.44 m (8.0 ft) above the floor may have exposed terminals.  a. Next to the shop foreman's desk there were two receptacles with live exposed parts from due to missing cover plates. Employees were exposed to a contact with live electrical parts and a fire hazard.

29 CFR 1910.305(b)(1)(ii): Unused openings in cabinets, boxes, and fittings shall be effectively closed.  a. Under the mezzanine, in the breaker box a circuit breaker was missing and not blanked. Employees were exposed to a contact with live electrical current, arc flash, and arc blast hazard.  b. In the tire cage area, the 220 Volt cabinet beside the sink had an unused opening (punch out) on the left side, thereby exposing employees to a contact with electrical current hazard, arc flash and arc blast hazard.
