29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii): a) Employees were exposed to the hazard of falling from an elevation of approximately 2 stories while working from scaffolding platforms that were not fully planked. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b): a) Employees were exposed to the hazard of falling while performing roof sheathing carpentry work from a pitched roof without the use of fall protection. b) Employees were exposed to the hazard of falling while performing carpentry work where there were window and wall openings at the second story level of the building. Caterpillar Carpentry Inc., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501 (b)(13) which was contained in OSHA inspection number 931711, citation 1, item 2, and was affirmed as a final order on September 12, 2013, with respect to a worksite located at 159 Abingden Ave., Kenilworth, Illinois. No abatement certification or documentation required for this violation.
