29 CFR 1910.22(a)(2): The floor of every workroom was not maintained in a clean condition. This violation was observed on or about April 30, 2014, in areas such as, but not limited to the area of the SawStop #1 table saw in the build crews area and around the dust collector in the same area, explosive dust was on the floor and around the machines where sawing operations and dust collection were performed exposing employees to the hazards of dust explosions.

29 CFR 1910.305(g)(1)(iv)(A): Flexible cords were used as a substitute for the fixed wiring of a structure: The employer did not ensure flexible electrical cords are not used as fixed wiring for a structure, such as, but not limited to the following: a) This violation was observed on or about April 30, 2014, in the builders area, the SawStop #1 table saw, the power cord was plugged into an extension cord that ran along the floor then into another extension cord and into an outlet on the wall exposing employees to the hazards of electrical shock. b) This violation was observed on or about April 30, 2014, ripsaw area, the Lobo ripsaw, the power cord was plugged into an extension cord that ran along the floor and into an outlet on the wall exposing employees to the hazards of electrical shock. c) This violation was observed on or about April 30, 2014, panel area, an orange extension cord was hardwired into the 2 duplex outlet mounted on the work bench exposing employees to the hazards of electrical shock.

This violation was observed on or about April 30, 2014, 2011, such as, but not limited to, the following: 1) The plug on the cord from the shop fox dust collector had the outer insulation pulled away from the plugs strain relief. 2) The orange extension cord from Saw Stop #1 plugged into another orange extension cord had the outer insulation pulled away from the plugs strain relief. 3) Black electrical cord from rip saw to dust collection system had the outer insulation pulled away from the plugs strain relief.
