29 CFR 1926.501(b)(2)(i): Each employee who was constructing a leading edge 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems, safety net systems, or personal fall arrest systems.       a) 2nd floor formwork deck:       An employee was securing formwork deck at a leading edge and was exposed to falling approximately 12 feet without fall protection. On or about 04/29/14.       NOTE:  THE EMPLOYER IS REQUIRED TO SUBMIT CERTIFICATION OF ABATEMENT FOR THIS ITEM.  FAILURE TO COMPLY MAY RESULT IN AN ADDITIONAL PENALTY OF $1,000 AS PER 29 CFR 1903.19.

On or about 04/29/14.       NOTE:  THE EMPLOYER IS REQUIRED TO SUBMIT CERTIFICATION OF ABATEMENT FOR THIS ITEM.  FAILURE TO COMPLY MAY RESULT IN AN ADDITIONAL PENALTY OF $1,000 AS PER 29 CFR 1903.19.
