29 CFR 1910.305(b)(1)(i): Openings through which conductors entered cutout boxes, cabinets, or fittings were not effectively closed: JOB SITE: 202 Lewers Street, Honolulu, HI 96815, on or before 4/30/2014, at this facility, located in the laundry dryer area, in the rear of the dryers, two (2) electrical cutout boxes with live exposed wires were not effectively closed. The boxes were missing knockout covers. The workers were exposed to electrical shock or burn hazards.
