29 CFR 1910.136(a): The employer did not ensure that each affected employee uses protective footwear when working in areas where there is a danger of foot injuries due to falling or rolling objects, or objects piercing the sole, and where such employee's feet are exposed to electrical hazards:  (a) Fabrication Area - The employer did not ensure that an employee used protective footwear when working in areas where a danger of foot injuries due to falling slabs and pieces of stone was present.    In accordance with 29CFR 1903.19(c), abatement certification is required for this violation (using CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement or other written records.

29 CFR 1910.178(a)(5): The powered industrial trucks was equipped with front-end attachments other than factory installed attachments, however the employer did not request that the truck be marked to identify the attachments and show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered:    (a) Fabrication Shop and Storage Areas - The Yale Y3 and Caterpillar 5 forklift trucks with front-end attachments (boom lift attachment) other than factory installed attachments did not have markings on the trucks to identify the attachments and show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered.     In accordance with 29CFR 1903.19(c), abatement certification is required for this violation (using CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement or other written records.

No abatement certification or documentation is required for this item.
