29 CFR 1926.501(b)(1) Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: A. The Velotta Company worksite located at I-475 & Ft. Meigs Rd., Perrysburg, Ohio: On or about April 30, 2014, the employer did not ensure employees were protected from fall hazards while cleaning the southbound bridge deck. The exposed employees were exposed to a fall distance of approximately 19.00 feet on the median side of the barrier wall. B. The Velotta Company worksite located at I-475 & Ft. Meigs Rd. (Railroad Overpass), Perrysburg, Ohio: On or about April 30, 2014, the employer did not ensure employees were protected from fall hazards while removing the parapet wall on the southbound bridge deck. The exposed employee was exposed to a fall distance of approximately 23.00 feet on the median side of the barrier wall.

29 CFR 1926.502(d)(8): Horizontal lifelines were not designed, installed, and used, under the supervision of a qualified person, as part of a complete personal fall arrest system, which maintains a safety factor of at least two: A. The Velotta Company worksite located at I-475 & Ft. Meigs Rd., Perrysburg, Ohio: On or about April 30, 2014, the employer did not ensure the horizontal lifelines installed on the jersey barrier were designed to be used as part of the fall protection system. The horizontal lifeline installed had deficiencies including but not limited to: no design criteria, inadequate clamps and clamp spacing, and unknown safety factors.
