29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: a. 1018 Beaver Dam Road, Braselton Court Apartment, Hoschton, GA (Jobsite) - Employees were exposed to fall hazard. The employees were not utilizing a means of fall protection, by not tying off to anchorage points, while working and walking approximately 9 feet above ground level on the roof, Installing roof sheathing along the upper level, on or about 05/01/14.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to upper landing surfaces, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladders were used to gain access: a. 1018 Beaver Dam Road, Braselton Court Apartment, Hoschton, GA (Jobsite) - Employees were exposed to fall hazard. Employees were using portable ladders that did not extend 3 feet above the upper landing while accessing the ladder to get to the roof of the residential apartment complex, on or about 05/01/14.
