29 CFR 1910.134(c)(1):   A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use:  On or about May 2nd, 2014:  Employees are required by the employer to wear an organic vapor half-facepiece respirator while painting in the spray booth. The employer did not establish and implement a written respiratory protection program for this required use of respirators.  Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).
