29 CFR 1910.37(b)(4): Signs were not posted along the exit access indicating the direction of travel to the nearest exit and exit discharge when the direction of travel to the exit or exit discharge was not immediately apparent: (a) On or about May 1st, 2014, plant employees were exposed to possible fire and/or smoke inhalation hazards due to the existence of a number of areas within the plant where the exit or path to the exit was not immediately apparent and was unmarked. Abatement certification is required for this Item. Use the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET for this purpose. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1,000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by points of operation, ingoing nip points, rotating parts, or flying chips and sparks: (a) On or about May 1st, 2014, employees were exposed to possible finger or hand injuries, including amputations, while working with and around the OxyDry Sheeters (Press 1 and Press 9) due to unguarded belts and pulleys (in-running nip points) across the open tops of the machines and parallel rotating cam-type shafts (containing nips at each cam's point of operation) arrayed slightly below the belts and pulleys. Abatement certification is required for this Item. Use the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET for this purpose. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM.
