29 CFR 1926.451(e)(1): When scaffold platforms are more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface shall be used: a. On or about April 30, 2014, at the above addressed jobsite, an employee accessing/egressing the ladder jack scaffold work platform was exposed to falls of approximately 25 feet due to the platform being positioned on the back of the ladders which required the employee to crawl over the platform from the support ladder.

On or about April 30, 2014, at the above addressed jobsite, an employee working from a ladder jack scaffold platform was exposed to falls of approximately 25 due to the fact that his lifeline/lanyard was attached to the building by two 16 penny nails which had been clinched over the 3/8" rope.
