Leighton Ave, Lincoln, NE: The employer is failing to ensure that flexible cords used by employees were protected from damage caused by an all terrain forklift. During the inspection of the jobsite flexible cords were found buried in mud having been run over by the New Holland all terrain forklift.

29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws: Aspen Builders jobsite 9215 E. Leighton Avenue, Lincoln, NE: The employer is failing to ensure that flexible cords were provided adequate strain relief. During the inspection a black flexible cord plugged in to a 220v temporary power outer was used to energize a Doolittle trailer. The flexible cord was not provided adequate strain relief in that the outer insulating jacket was pulled out of a junction box in the run of the cord exposing the inner conductors of the cord.

29 CFR 1926.405(j)(2)(ii): A receptacle installed in a wet location was not designed for the location: On the jobsite 9215 E Leighton Avenue, Lincoln, NE: The employer is failing to ensure that electrical receptacles used by employees to energize tools and or equipment were designed for outdoor / wet location use. Employees were using a homemade outlet box provided by the company to an energized yellow flexible cord. The homemade outlet box was made with 2 steel double gang boxes screwed together which housed 4- 15 amp, double outlet receptacles wired in parallel with flexible cord. The homemade outlet box was made with multiple components that are not designed for use in wet locations such as but not limited to outdoors on a construction site in that it was not watertight to prevent electrical short.

The employer allowed employees to operate an all terrain forklift to move materials on the jobsite none of the operators had received training.
