29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws:  a) On or about May 2, 2014, at the above addressed job site, an employee was exposed to shock and electrocution hazards while using an energized electrical extension cord to power a floor fan, that had a receptacle that lacked strain relief.

29 CFR 1926.405(a)(2)(ii)(I): Flexible cords and cables used for temporary wiring were not protected from damage:  a) On or about May 2, 2014, at the above addressed job site, an employee was exposed to shock and electrocution hazards while using a floor fan that had been powered by an energized electrical extension cord that had been placed across the road and exposed to vehicle traffic, that had damaged outer insulation jacket at the receptacle.
