29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502: (a) Roof, residential home located at 2726 East Field Road, Smyrna, GA 30080. Employees were exposed to fall hazards while engaged in residential roofing activities on a 5/12 pitched roof at eave heights between 8-feet, 9-inches and 12 feet with no fall protection system in place.

29 CFR 1926.552(a)(1): The employer did not comply with the manufacturer's specifications and limitations applicable to the operation of all hoists and elevators. a) North side, residential home located at 2726 East Field Road, Smyrna, GA 30080. Employer allowed employees to use the TranzSporter (Model LH-200) Platform Hoist as a ladder to access the roof. The Manufacturer's General Safety Instructions state that this hoist should never be used as a ladder.
