29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:     a) Lot 51, 2559, Marietta GA. 30062 - Scaffold: The onsite supervisor was exposed to head injury by being struck by the bricks while throwing the bricks to load the upper level of the scaffold. He was working from the ground level under the scaffold without wearing a hard hat.

29 CFR 1926.451(b)(1)(i): Each platform unit (e.g., scaffold plank, fabricated plank, fabricated deck, or fabricated platform) was not installed so that the space between adjacent units and the space between the platform and the uprights were no more than 1 inch (2.5 cm) wide, except where the employer can demonstrate that a wider space is necessary.     a) Lot 51, 2559, Marietta GA. 30062 - Scaffold: Employees were exposed to trip and fall hazards of up 13 feet 7 inches while working from a fabricated frame scaffold that was not fully planked.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, sturcure, personnel hoist, or similar surface was not used.     a) Lot 51, 2559, Marietta GA. 30062 - Scaffold: Employees were exposed to falls of up to 13 feet 7 inches while climbing up and down the scaffold frames because safe access to the upper levels of the scaffold was not installed. The ladder was onsite sitting approximately 25 feet to the right of the scaffold with other scaffolding components.

29 CFR 1926.451(h)(1): Each employee on a scaffold was not provided with additional protection from falling hand tools, debris, and other small objects through the installation of toeboards, screens, guardrail systems, or through the erection of debris nets, catch platforms, or canopy structures that contain or deflect the falling objects.  For falling objects too large, heavy or massive to be contained or deflected, the employer did not place and secure such potential falling objects away from the edge of the surface from which they could fall.     a) Lot 51, 2559, Marietta GA. 30062 - Scaffold: Employees were exposed to struck-by hazards to the torso while working from the lower levels of a fabricated frame scaffold without any structure installed that would stop falling debris such as but not limited to the bricks that were being loaded within 6 inches from the top platform edge.
