This condition exposed employees to caught-in hazards.  (b) Boiler Room: On or about April 30, 2014 and at times prior thereto, two (2) inches of the keyed rotating horizontal shaft from the motor to the floor-mounted pump of HWP-2 was not completely encased. (c) Boiler Room: On or about April 30, 2014 and at times prior thereto, one and one quarter (1.25) inches of the keyed rotating horizontal shaft from the motor to the floor-mounted pump of CWP-1 was not completely encased. Snap switches mounted in boxes did not have faceplates installed so as to completely cover the opening and seat against the finished surface:  Room 1617, Oxygen Storage: On or about April 30, 2014 and at times prior thereto, the light switch for the room did not have a coverplate. This condition exposed employees to electrical shock and burn hazards from contact with the 120 volt circuit.

29 CFR 1910.303(g)(1)(ii): The required working space about electric equipment rated 600 volts, nominal, or less to ground, was used for storage:  (a)  Ambulance Bay: On or about April 30, 2014 and at times prior thereto, the working space of the Fuel Oil Polisher and Garage TCP was occupied by a patient Hill-Rom litter and Werner step ladder.  This condition restricted employee access to the control switches.  (b)  Central Supply, Room 1613: On or about April 30, 2014 and at times prior thereto, the working space of the Load Sterilizer switches was occupied by a medical supplies.  This condition restricted employee access to the control switches.  (c)  Loading Dock: On or about April 30, 2014 and at times prior thereto, the working space of the of the switches located adjacent to the exit was occupied by a hand cart and battery charger.  This condition restricted employee access to the control switches.  (d)  Boiler Room: On or about April 30, 2014 and at times prior thereto, the working space of the LOAD HWP4 disconnect was occupied by four barrels of boiler treatment chemicals.  This condition restricted employee access to the control switch.
