29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury: a) On or about 05/05/14, on the flat roof of building at 237 Park Ave., Lockport, NY: Five employees engaged in construction work were not trained to recognize potential hazards on this type of jobsite including but not limited to falls from roofs, ladders, skylights, stairs & scaffolds, electrical hazards, unguarded tools/machines, fire hazards, and the need for personal protective equipment to protect them from various hazards. They also were not trained in the applicable OSHA regulations to control or eliminate these hazards. ABATEMENT CERTIFICATION REQUIRED

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. a) On or about 05/05/14, on the flat roof of building at 237 Park Ave.,Lockport, NY: Five employees were exposed to a potential fall of approx. 17 ft. from the edges of the roof while engaged in roofing work. No fall protection system was utilized. NO ABATEMENT CERTIFICATION REQUIRED
