29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:    (a) Lancaster Container, Inc., 27 Penn Street, Washington Boro, Pennsylvania and 100 Bridge Street, Columbia, Pennsylvania - Employees performed servicing and maintenance activities on a variety of machinery and equipment, including, but not limited to, mechanical power presses, seam welders, and conveyors, and the employer did not develop and implement machine specific energy control procedures, exposing employees to caught-in, struck-by and other hazards resulting from unexpected energization, on or about May 5, 2014.    Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.

29 CFR 1910.147(c)(7)(i)(A): Authorized employees did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control:  (a) Lancaster Container, Inc., 27 Penn Street, Washington Boro, Pennsylvania - Employees performed servicing and maintenance activities on a variety of machinery and equipment, including, but not limited to, mechanical power presses, seam welders, and conveyors, and authorized employees did not understand when to apply lockout/tagout (for example, authorized employees did not use lockout/tagout during some set up activities), exposing employees to caught-in, struck-by and other hazards resulting from unexpected energization, on or about May 5, 2014.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.

29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees:  (a) Lancaster Container, Inc., 27 Penn Street, Washington Boro, Pennsylvania - To accommodate the size of pail or container in production, a machine operator adjusts the plates on the transfer table that moves the metal sheets to the bender and then the seam welder.  When doing this set up task, the machine operator activates the emergency stop rather than using lockout/tagout, exposing the employee to caught-in, struck-by and other hazards resulting from unexpected energization, on or about May 5, 2014.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.
