29 CFR 1910.304(g)(5): The path to ground from circuits, equipment, and enclosures was not permanent, continuous, and effective: a) Outside Mushroom Room, on or about 5/6/14: A green extension cord used for charging the pallet jacks was not grounded in that the ground pin was missing from the plug end. b) Outside Wet Room, on or about 5/6/14: An orange extension cord used for charging the pallet jacks was not grounded in that the ground pin was missing from the plug end. c) Receiving Dock, on or about 5/6/14: An orange extension cord used for charging the pallet jacks was not grounded in that the ground pin was missing from the plug end. Abatement documentation must be submitted for these items. The J-20 Corporation was previously cited for a violation of this Occupational Safety and Health standard, which was contained in OSHA inspection number 314344250, Citation number 1, Item number 2 and was affirmed as a final order on 5/18/10, with respect to a workplace located at 900 Wolf Street, Syracuse, NY 13208.
