David's Heating Co., Ltd., 12220 YMCA Drive, Edinboro, PA  16412, on or about May 5, 2014 and times prior thereto:  Employee utilize DuroDyne DDS-181 Duct Closure Mastic for the installation of ductwork.  The MSDS for the DuroDyne DDS-181 Duct Closure Mastic was not available on-site for employees to access.

David's Heating Co., Ltd., 12220 YMCA Drive, Edinboro, PA  16412, on or about May 5, 2014 and times prior thereto:  Employee worked around and/or with chemicals that included but were not limited to  DuroDyne DDS-181 Duct Closure Mastic.   Employee hazard communication training did not cover the specific physical and health hazards of the chemicals.   In addition, the hazard communication training did not cover the means and methods of protection against the hazards posed by the chemicals.
